Prescription
Prepayment Certificates
To obtain
a Prescription Prepayment Certificate you will require an application
form FP95 from the Health Advice Centre (you can use the Information
Request Form), Primary Care Trust, a main Post Office or from
a pharmacist. The form tells you what to do.
All Prepayment
Certificates are now issued by the Prescription Pricing Authority.
You can ring their Order Line on 0845 850 0030 (have your
credit or debit card ready) or send the application form with
your credit/debit card details or with your cheque or postal order

Cheques/Postal
Orders must be made payable to the "Prescription Pricing
Authority" and crossed "Payee Only". Please write
your postcode on the back of your Cheque/Postal Order.
Backdating:
The Prepayment
Certificate will start from the date the Prescription Pricing
Authority receives the application form unless you put a different
start date. This cannot be more than one month earlier, or
one month later, than the date the PPA receives the form.
Refund of Prescription Charges
You must apply
for a new PPC in good time, otherwise you will have to pay charges
when your old PPC runs out.
If you have
to pay a prescription charge while you are waiting for your PPC,
you cannot get a refund unless you get an NHS receipt form (FP57)
and your new certificate covers the date you paid
the charge. When you apply, you can ask for your PPC to be backdated
up to one month.
Refunds
of Prescription Prepayment Certificates (PPCs)
You can claim
a full refund of the cost of a 4 or 12 month PPC if, within the
first month, you become entitled to free prescriptions. You must
claim this refund within 4 months of date the PPC started.
You can claim
a partial refund of the cost of a 12 month PPC if, within months
2-4 of the PPC, you become entitled to free prescriptions.

For a refund
of Prepayment Charges a claim needs to be made in writing, giving
details of the reason for a refund, to:

The original
Prepayment Certificate will need to be returned with the request
for refund. In addition:

For refunds on benefit or tax credit grounds a copy of the award
notice will need to be sent with the letter

For refunds on Medical or Low Income grounds the PPA will have
the necessary proof available.
