Medical Exemption
from NHS Prescription Charges
If you suffer
from certain medical conditions you are entitled to free NHS prescriptions.
But you need an exemption certificate form FP92A to say that you
do not have to pay. You can obtain this form from Rotherham Health
Advice Centre by contacting us or by filling in the Information
Request Form or by contacting your GP. You will then need
to fill in Parts 1 and 2 of the form and ask your GP or hospital
doctor to complete Part 3. Your GP will then send the form to
the Prescription Pricing Authority who will issue the medical
exemption (Medex) card .
The medical
conditions are as follows:
· permanent
fistula (for example caecostomy, colostomy, laryngostomy or ileostomy)
requiring continuous surgical dressing or requiring an appliance
· epilepsy
for which you need continuous anti-convulsive therapy
· diabetes
mellitus except when treated by diet alone
· myxoedema
or other conditions which require supplemental thyroid hormone
· hypoparathyroidism
· diabetes
insipidus or other forms of hypopituitarism
· forms
of hypoadrenalism (eg Addison's disease) for which specific substitution
therapy is essential
· myasthenia
gravis
· a
continuing physical disability which means you cannot go out without
the help of another person - temporary disabilities do not count
even if they last for several months
The card will
start from the first day of the month that the Prescription Prescribing
Authority receives the application form. Your exemption card will
be posted to you.
Medical exemption
cards normally last for five years and then need to be renewed.
You may receive a reminder that your card needs renewing, but
if not, it is your responsibility to ensure that it is renewed.
